The PFAnetwork (a program of PFCCpartners) is a community of patients, family caregivers, and community leaders reinventing healthcare through authentic partnerships.
Our diverse lived experiences shape healthcare policies, programs, and services that truly meet the needs of patients and families. Authentic partnership is the cornerstone of our mission. Lived experience is expertise. We believe patients and families are not “add-ons” to healthcare. We live the outcomes of every decision, and when we are engaged early and meaningfully, services become more responsive, equitable, and trusted.
Building and Strengthening Community Connections
We create spaces where Patient and Family Partners (PFPs) can:
- Connect, grow, and build leadership confidence
- Strengthen skills for impactful engagement in diverse opportunities
- Co-design services and structures alongside professionals
- Move systems from consultation to true collaboration
We also support healthcare teams in strengthening their readiness for partnership, because meaningful engagement requires capacity on both sides.
Making a Tangible Difference
We see impact when partnership is embedded, including:
- Patient Family Partner leadership at governance and advisory tables
- Skills exchanges and learning for emerging PFPs
- Co-designed initiatives that improve patient and family experiences
We are a small but mighty team of passionate PFPs ourselves, using our lived experiences to empower others and model authentic partnership. And, we have FUN doing it!
Partnership in Action
As Artificial Intelligence (AI) tools are introduced into care settings and healthcare partnership activities, the PFAnetwork is preparing PFPs to engage early and confidently alongside health system leaders in these conversations. Through co-designed skill-building activities, we are strengthening partnership readiness. For example, our PFP leaders are sharing best practices around the productive use of prompting AI for successful outcomes. Our community also shares real life experiences about what has worked well in diverse health care settings and initiatives. By building shared knowledge and confidence, we are helping ensure innovation does not outpace inclusion.
